STATE OF ILLINOIS) SS. [scilicet]
being duly sworn, on oath deposeth and says, that he attended as witness in the case wherein Joseph Smith is plaintiff plaintiff, and & <​are​> defendants for five days at the September Term, A. D. 1843, of the Circuit Court of said at the instance of said plaintiff & that he resides in 160 miles from the place of holding this court
Subscribed and sworn to before me this 10 day of May A. D. 1844
