[TO] WHOM IT MAY CONCERN:
[This certifies that] [Willia]m Martin—— has been received into [the church of Jesus Christ of Latt]er Day Saints, organized on the sixth of April, in the year of our Lord [one thousand eight hundred and thir]ty, and has been ordained an Elder—— according to the [rules and regulations of said church;] and is duly authorised to preach the gospel, agreeably to the authority [of that office.]
[Given by the direction of a gener]al conference of the authorities of said church, assembled in , Ill. [on the sixth of April, in the year of ] our Lord one thousand eight hundred and forty.
