This document consists of three leaves. The first two leaves are presented here as a single page based on the continuation of handwriting and column lines. The third leaf is presented as a separate page and repeats information found on the bottom leaf of the first page. There are no clear indicators that any of the three leaves were physically attached, but the second and third leaves feature similar wear patterns and water damage that suggest they were stored together.

A nearly perfect circle on the verso of the second leaf and a matching circle on the recto of the third leaf may indicate there was an adhesive wafer holding these two leaves together.
